  The process has been easier than I thought it would be and the questionnaire is simple and straightforward. The online chat is very helpful for any questions you may have as you fill out the forms. So far so good, but watch out for the access fee if you take too long to finish up the paperwork... the charge is over $36 for access after a certain time. I never actually authorized this charge but it was charged to the credit card used to start the procedure. There is also a $32 printing charge.
